Aşağıdaki başvuru formunun eksiksiz doldurulması gerekmektedir.
Henüz Covid-19 Aşısı olmamış öğrenci ve misafirlerimizin "Aşı Kartı Doğrulama Adresi" kısmına, "PCR Testi" yazmaları gerekmektedir. "PCR Testi" yazan öğrenci ve misafirlerimizden, tören günü kampüsümüze girişte, son 3 gün içerisinde yaptırmış oldukları PCR Testi talep edilecektir.
* "Hes Kodu" bölümüne, Hayat Eve Sığar uygulamasında yer alan Hes Kodunuz yazılacaktır.
* "Aşı Kartı Doğrulama Adresi" bölümüne, Aşı Kartınızda yer alan QR Kodu okuttuğunuzda yönlendirilen, "covidasidogrulama.saglik.gov.tr" ile başlayan sayfanın linki kopyalanacaktır.

Students and guests who have not yet been vaccinated against Covid-19 are required to fill in "PCR Test" in the "Vaccination Card Verification Address" section. Our students and guests who fill in "PCR Test" will be required to have the PCR Test which has done in the last 3 days upon entry to our campus on the day of the ceremony.
* In the "HES Code" section, your HES Code that is included in HES application will be fill in.
* The link of the page starting with "covidasidogrulama.saglik.gov.tr", which is directed to the "Vaccination Card Verification Address" section when you scan the QR Code on your Immunization Card, will be copied.
